Deposit Fee to Confirm Your Appointment:

  • A minimum deposit of $100is required for all Consultations and/or treatments at the time of scheduling to confirm and hold your clinic appointment. This deposit will go toward your consult fees at the payment of the balance at the end of your appointment. Appointments without a deposit will be canceled within 48 hours of scheduling.

Cancellation Policy:

Cancellations are sometimes unavoidable, but we ask for at least 24-hour notice (calls, email are preferred) as a courtesy to other clients who are waiting for an appointment and out of respect for Dr. Berberian’s time.

  • Deposits of $100 are non-refundable for cancellations made less than 24 hours from your scheduled appointment time (emergencies are considered), and for those who do not care to call to cancel.
  • Patients that have consults scheduled, will just have the consult, not the actual procedure for that day and time unless arranged prior to the date.
  • A patient that is more than 15 minutes late without notification, unfortunately, will not be seen.
  • Patients that do NOT SHOW, without some sort of courtesy notification. Will not be seen.
  • Patients that have more than(3)NO- SHOWSwithoutnotification will be dismissed from this practice.
  • Groupon patients will lose treatment for a No-Show, from their prepaid package.
  • Patients are called via computer messaging the evening prior to your appointment you MUST CONFIRMAgain, you are called, emailed, text messaged 2 hours before your actual appointment. If there is NO CONFIRMATION via email, text message, voice mail, and voice. You will be taken off the schedule, and you will not be seen.

*** Your understanding is appreciated. This is a small boutique practice. Everybody has busy lives, but for the ones on time, they should not be inconvenienced.
